(I use Sonar PE, forgive me if I'm wrong about that). You will have to route the audio within Sonar, but set that config up, and keep it, you should only have to config once.

I use that, and with it, you could stream audio in straight though channel by channel, rather than bottle-necking though firewire, plus you would be getting a huge upgrade in sound from the card you are currently using. However, that could get very expensive for 12 ins. Prob be the best to find a card for your pc that has as many inputs, as your mixer has outputs.
